Oral Hygiene × SARS-CoV-2

The British Dental Journal recently found that poor oral hygiene may be linked to more severe cases of COVID-19 because of the harmful bacteria found in mouths that have not been properly taken care of.

While the mouth has always been known as a gateway to the rest of the body, giving it the ability to cause problems in other areas, it is now found that poor oral hygiene can cause respiratory infections, making COVID-19 stronger.

The good news is that the best defense, in this scenario, is to follow good oral practices, like flossing, brushing and using mouthwash.

COVID-19 continues to be deadly, but there does appear to be some sort of link in more than half of fatal cases.

According to the British Dental Journal, “More than 80% of COVID-19 patients in ICUs exhibited an exceptionally high bacterial load, with more than 50% of deaths exhibiting bacterial superinfections.”

Even though COVID-19 is transferred virally, complications like pneumonia and acute respiratory distress might be caused by bacterial superinfection, which begins in the mouth.

The study says, “We recommend that oral hygiene be maintained, if not improved, during a SARS-CoV-2 infection in order to reduce the bacterial load in the mouth and the potential risk of a bacterial superinfection.”

Again, hygiene can be maintained by brushing, flossing and using mouthwash, but oral-care probiotics can also offer protection.

Oral-care probiotics are a specialized type of probiotic formulated to repopulate the oral cavity bacteria, which battles harmful bacteria that could lead to cavities, gingivitis and periodontal disease.

Dr. Eric Goulder, founder of the Heart and Stroke Prevention Center of Central Ohio, said he thinks heart health is also determined by oral health. His team uses ProBiora, which supports health in teeth and gums.

“We think everyone should be extra careful during the pandemic, and oral-care probiotics are a great way to help keep the oral cavity in balance 24-7,” Dr. Goulder said.

Senate called on to include $200 billion for charities in relief package

A coalition of nonprofit groups is calling on the U.S. Senate to include a temporary emergency stimulus in its next pandemic relief package. The proposal would unlock $200 billion in charitable funds to assist charities overwhelmed by the pandemic, with updates to the laws governing private foundations and donor-advised funds (DAFs). The proposal would release more of the estimated $1.2 trillion they currently hold by increasing required distributions to 10 percent annually for three years.

“Nonprofits need emergency help right now. Millions of nonprofit jobs have been lost, one-third of them in health care. Up to 120,000 nonprofits are shutting down completely,” said Scott Wallace, co-chair of the Wallace Global Fund, a private foundation that committed to spend 20 percent of its own endowment in 2020. “We urge Congress to enact an Emergency Charity Stimulus to force philanthropies to increase their support for nonprofit organizations – immediately, urgently, and temporarily, to allow time for deployment of a vaccine and economic recovery.” 

“We are collectively facing the most dire moment that many of us have seen in our lifetimes, and it is likely the tip of the iceberg in terms of the challenges that await us as a society and a planet,” said Aileen Getty, founder and president of the Aileen Getty Foundation and granddaughter of billionaire J. Paul Getty.

“While some foundations and donors are stepping up at this moment, others continue to treat the five percent payout as a ceiling not a floor,” said Chuck Collins, director of the Charity Reform Initiative at the Institute for Policy Studies. ““Donors have already taken the tax break for these contributions. Congress needs to raise the bar for those donors who haven’t figured out this is no time to sit on your treasure.”

Led by the Charity Reform Initiative of the Institute for Policy Studies, Patriotic Millionaires, and the Wallace Global Fund, the groups first proposed the idea in May with a letter to Congress. The letter has now been signed by almost 800 philanthropists and leaders of foundations as well as several thousand nonprofit leaders and staff.

The proposal calls for a temporary doubling of private foundation payout from 5 percent to 10 percent for three years and would establish a similar 10 percent payout for donor-advised funds (DAFs) that currently have no mandate.

Researchers at the Institute for Policy Studies estimate these policies would unleash an estimated $200 billion in additional charity funds over three years, with no additional cost to taxpayers. The independent nonprofit sector is part of the front-line response to the pandemic and other natural disasters. The sector employs 12 million workers or more than 10 percent of the private workforce.

The “Magic” of the Disney College Program

“The Magic Kingdom is now open!” a loud voice blares over the intercom at the entrance of the most famous park at Disney World. Hoards of people rush towards the small gates to journey into the land of magic. Upon entering, the magic hits like a wall with Mickey Mouse balloons and a Main Street lined with buildings that look like they were pulled from a storybook. This all leads up to the glistening masterpiece that is Cinderella’s castle. 

But how magical is this experience when you have to do it daily as a burnt-out college student working to make ends meet? For thousands of college students, this is their daily life, but the magic gets dull with each screaming child and cranky parent they encounter. Does the magic truly vanish while working for the Disney College Program, or is all the hassle worth the enchantment that encompasses Disney? 

What it Takes to Work for the Mouse

Although it may seem like no one would be begging to work in a theme park, the Disney College Program gets thousands of applicants every year and only ends up accepting less than 20% of those students. The Disney College Program offers programs at both Walt Disney World in Florida and Disneyland Resort in California. There are five basic qualifications students must meet before they should begin to apply.

According to the Disney Program website, students that hope to apply to the program must be “enrolled and taking classes at an accredited program or institution” at the time they hope to apply. Students must have already completed at least one semester of classes or have graduated within 12 months. The program is specifically designed for undergraduate students, but graduate students are able to apply. Some individual universities have special requirements for students to meet such as a specific GPA, so students should meet with an advisor at their school before applying. 

The program also requires all applicants to be at least 18 years old by the time the program starts as well as possess an unrestricted work authorization. Finally, students that have done the program before must wait at least four months from their departure date to return to the program again. 

However, the program requirements go deeper than this. Since the program is so competitive applicants need to do their best to stand out. The first step of the application process is a general questionnaire that’s similar to many basic job applications. After passing this step, applicants move on to a more in-depth web interview. Finally, a phone interview makes the final decision if someone is selected for the program. 

The Captivating Cast Member Positions

There’s a variety of positions available for students that register for the program. The jobs range from working in the parks to working in the hotels and are assigned by random or based on applicants’ prior experience. These positions are no walk through wonderland, they’re full-time positions and students need to be available to work days, nights, weekends and holidays. The wage depends on the position, but most of the employees only make around $9 an hour or a similar amount close to minimum wage. The paychecks certainly aren’t fit for royalty. 

One past cast member who was a part of the Disney college program, Rebecca Condon, worked Merchandise at the Emporium on Main Street in the Magic Kingdom. This retail experience allowed her to become a manager at Lilly Pulitzer at the young age of 22. Northeastern alumni, Kayla DiPilato also participated in the program as a seater at Be Our Guest, a themed restaurant in the Magic Kingdom. She believes she received this position because of her prior experience as a hostess at Top of the Hub in Boston. 

As both of these jobs seem to capture the whimsical essence of Disney, many positions in the program are not as sought over. Some roles such as custodial or food prep, are minimum wage jobs that can be found at most basic establishments all around the country. However, what would be a part-time job in a fast-food restaurant or a business turns into hours on hours in an amusement park to make ends meet. Depending on which job an applicant is selected for could determine whether they love or hate the program. Although, one person’s job nightmare could be a dream come true for somebody else. 

Not a Castle Nor a Carriage 

The housing and transportation for the program has not received stellar reviews from past cast members. Members of the Disney College Program are housed in apartments and rent is taken weekly from their paychecks. Rent can cost anywhere from $114 to $205 a week depending on what housing a person is placed in. Many times cast members have to share a room as well as sharing the apartment with a few other workers. The rules of housing are apparently incredibly strict, with restrictions against alcohol as well as overnight guests of the opposite gender. 

“My least favorite part of the job was the apartments they housed us in,” said Rebecca Condon. “My apartment was the oldest Disney property and had tons of issues. My toilet overflowed 7 times during the program because of bad pipes in the wall to the point where it flooded our whole apartment with about 2 inches of water.” The rent might be cheap, but in the end you get what you pay for. 

If students were unable to bring their own car, they had to rely on the transportation provided by Disney. The college program in Orlando provides a shuttle service to help transport cast members, but the Anaheim program only provides cast members with a free city bus pass. Although the shuttle sounds like a convenient option, it was much less practical than having a car on the property. 

“It is unbelievable how they are able to transport thousands of us,” expressed Condon. “Although, with that being said, it was really hard because I would have to leave two hours before my shift to make sure I got there in time and I often wasn’t home until two to three hours after my shift.” After an incredibly long day of working in a busy park, a two-hour commute is much longer than anyone would want to endure. 

The unreliable shuttle was one reason DiPilato decided to drive her car all the way from Massachusetts to use during her time in the program. “I knew how disastrous it would be to take the shuttle for commuting,” she said. “I also wanted to have the freedom to explore Orlando.”

Experience the Magic but Fight the Villains 

Disney has its perks as well as its downsides just as any normal day job does. DiPilato said her favorite part of the job was making magic for guests, especially for kids that were part of the Make A Wish program. However, she also recalled that families would often get hot and tired after a long day in the park and take out their frustrations on her.  

There were also a lot of strict rules such as never being allowed to point with one finger, never calling guests “people” instead of guests, not being able to have piercings besides one on your earlobes and not being able to wear too much makeup. 

“Once I got yelled at because a child threw his shoe into our moat. How was that my fault?” recalled DiPilato. “Although, I did get to meet Josh Gad in promotion of the Beauty and The Beast live-action movie that was set to come out at the time, so that was a super cool experience.”

Condon recalled one occurrence where the cast members got to experience an exclusive party for the cast members. “Disney opened up one of their water parks for cast members after hours,” she recalled. “They hired a DJ and catered with some of the best Disney Treats, especially the Mickey Bars!”

Every day working at Disney for the Disney College Program is a unique experience. DiPilato mentioned that guests would often sprinkle ashes of relatives within the rides and cast members would have to clean them up. “Yeah, that happened a lot, mostly in the Haunted Mansion,” she said nonchalantly. She also touched on the fact that kids would often get separated from their families, throw up randomly and scream… a lot. “Giving kids a magical experience is amazing, but it comes with so many more problems than would come with working strictly with adults,” she said. 

Is the Work Worth the Pixie Dust?

It takes a special and dedicated person to participate in the Disney College Program. Days are full of long hours of work and lots of cranky families. However, the perks and experience a cast member receives from the job will last a lifetime. 

“I absolutely loved the program and feel like I grew so much from it,” concluded Condon. “The skills I learned from working for this Fortune 500 company is something I carry around with me every day.” 

If you could walk through the streets of Magic Kingdom daily without it ever getting old, or eat a Mickey Ice Cream bar every day without ever getting sick of them, you could have what it takes to become a cast member. To many, it is the job of a lifetime to be able to play an important role in millions of children’s most magical memories and the free park entry doesn’t hurt either. 

If you can get over the job’s flaws, you could have Mickey Mouse as your coworker. And hey, don’t all jobs have their downsides?

New Nursing Home Cases In Midwest States Increase By More Than 400% Since September

AHCA/NCAL Calls On CDC To Give Long-Term Care Facilities The Highest Priority For Vaccine Distribution And On Congress To Replenish Emergency Funding 

The American Health Care Association and National Center for Assisted Living (AHCA/NCAL) represents more than 14,000 nursing homes and assisted living communities across the country that provide care to approximately five million people each year. Today, they released an updated report showing nursing homes in the U.S. have now experienced the worst outbreak of weekly new cases since last spring due to the community spread among the general population, surpassing previous peaks since the Centers for Medicare & Medicaid Services (CMS) started tracking cases in nursing homes.

Recent data released by Johns Hopkins University and the Centers for Medicare & Medicaid Services (CMS) show that with the recent spike in new COVID cases in the general U.S. population, weekly nursing home cases are also on the rise. According to Johns Hopkins University, weekly new COVID cases in the general U.S. population rose by 330 percent to 1,043,040 new cases the week of November 15. A correlating uptick in new cases in nursing homes occurred when cases in the surrounding community started rising back in mid-September.

As experts have repeatedly noted, COVID-19 cases in a surrounding community is a top factor in outbreaks in nursing homes. University of Chicago’s Tamara Konetzka, a nationally recognized expert on long-term care, recently said, “Trying to protect nursing home residents without controlling community spread is a losing battle.” Dr. David Grabowski, Professor of Health Care Policy, Harvard Medical School recently stated, “The strongest predictor of whether or not we’ll see cases in [a particular setting] is community spread.”

“Our worst fears have come true as COVID runs rampant among the general population, and long term care facilities are powerless to fully prevent it from entering due to its asymptomatic and pre-symptomatic spread,” stated Mark Parkinson, President and CEO of AHCA/NCAL.

“Our health care heroes are doing everything they can to prevent it from spreading further, but this level of COVID nationwide puts a serious strain on our workforce, supplies, and testing capacity,” said Parkinson. “Given the fact that our elderly population is the most vulnerable and the rising level of COVID across the U.S. shows no signs of stopping, it is paramount that the Centers for Disease Control and Prevention (CDC) provide the highest priority for the vaccine distribution to long term care residents and staff.”

During the week of November 15, nearly half (49 percent) of new COVID cases in nursing homes were from Midwest states with major spikes in the community spread in the upper parts of the region. As a result, the Midwest region saw more than a 400 percent increase in weekly COVID cases in nursing homes since mid-September.

After seven weeks of declining cases in nursing homes through mid-September, nursing home cases began to increase as nearly all 50 states started to see significant rising levels of COVID cases. New weekly cases in nursing homes grew by more than 177 percent nationwide between mid-September and the week of November 15.

The report also showed COVID-related deaths in nursing homes are starting to rise, crossing more than 2,000 residents lost the week of November 15—the first time since early-June. Nursing home residents are typically older adults with multiple chronic conditions, making them most vulnerable to COVID-19.

Residents of long term care facilities account for only seven percent of the nation’s cases, yet 40 percent of its deaths. While mortality rates decreased compared to the spring due to a better understanding of the virus, better treatments, and government resources to help reduce spread, as industry leaders predicted, the rising number of new COVID cases in facilities are resulting an increasing number of deaths.

“With millions of Americans failing to heed advice from public health experts and traveling during Thanksgiving, we are extremely concerned that this situation will only get much worse,” continued Parkinson. “At this point, long term care facilities desperately need public health officials at every level to take emergency steps to get control of the community spread and ensure our facilities have the resources they need, as well as for the CDC to make our residents and caregivers the top priority in distributing the vaccine in order to save thousands of lives.”

With record new COVID cases across the country, Parkinson said Congress must also prioritize frontline health care workers and long term care residents during the lame-duck session. Last week AHCA/NCAL released a list of actions that Congress should urgently take to help nursing homes and assisted living communities respond to the uptick in new cases.

Most of the $175 billion Provider Relief Fund provided by the CARES Act back in April has already been distributed and Parkinson said health care providers, including long-term care facilities, will need additional funds to continue the response to the COVID pandemic. The financial aid is crucial in helping long-term care facilities acquire personal protective equipment, conduct regular testing, and hire additional staff or reward current caregivers for their heroic efforts.

“Congress must fulfill its duty,” stated Parkinson. “Without adequate funding and resources, the U.S. is repeating the same mistakes made during the initial outbreak last spring and the major spike over the summer. We need Congress to prioritize our vulnerable seniors and their caregivers in long term care facilities, by passing another COVID relief package during the lame duck session on Congress.”
